My First Visit

Congratulations For Taking The First Step To Better Health!

We will guide you through each step of our 5 step process of becoming a new patient. Please feel free to reference this section, or our entire website at any time of your new patient process.

steps-1 We strive to make your first visit to our office a comfortable experience. Our receptionist will welcome you and ask you to complete our required forms. These forms include insurance/payment information, consent to treatment, and HIPAA patient confidentiality forms. Your assigned case manager will then spend time reviewing your fibromyalgia questionnaire and determining what initial course of testing will be necessary.

step-2 After discussing your history and symptoms, your case manager will give you a thorough physical examination, such as testing the musculoskeletal system, your reflexes, and other standard neurological, orthopedic, and physical examinations. This important information will allow the doctors to have a better understanding of your condition and how we can help you. If you needed, additional testing may be ordered.

It is standard for us to draw blood for essential blood tests during your first visit. By performing an examination and blood tests on the first visit, it allows us to begin treatment sooner. If you desire to only have a consultation with a case manager for your first visit, simply tell the receptionist when you call to schedule your appointment and we will arrange for this.

After all the information including exams, forms, and testing are complete, your case manager will sit down with you to go over the findings and appropriate treatment plan. Your treatment plan is constructed by an entire team of providers and is based on your individual condition.

Over 85% of our patients get greater than 70% relief from our programs within four months of treatment, however results do vary. Treatment plans vary and alterations are sometimes necessary based on an individuals' response to treatment early in the program. Your case manager will be available for discussion. 


Our office participates in many different insurance programs; for patients with no insurance coverage a payment plan can be approved.
